Atlanta
- You must report the injury in writing within 30 days of the incident. If you do not report it within
30 days, you may not be eligible for workers compensation benefits or payments - In Georgia you cannot choose to see your own doctor. You must go to a doctor that the company
designates. You may be able to choose a doctor from a panel of state approved doctors. You must

Austin
- In Texas you can only see a doctor within the company’s network. You cannot choose your own
doctor unless it is from the list of in network doctors. The treating physician must refer to all
specialists. Ask your case manager for the list. - You will not become eligible for workers comp pay until after you have missed 8 consecutive
days of work. - Your workers comp benefit will end after 104 weeks of receiving the pay.
- In Texas the minimum amount you can receive weekly is $176 and the maximum is $1,173.81 per
week. - You can request mileage reimbursement for medical treatments more than 30 miles one way. The
rate of pay is 65.5 cents per mile. If you would like to be reimbursed for your mileage request a

Baltimore
- You can choose to see your own doctor if they take workers compensation. You must tell your
case manager your doctor’s name and address so they can approve your visits. - You can claim mileage reimbursement. The rate is .655 cents per mile. If you would like to be

Dallas

Denver
- Per state law, you should report your injury in writing to the company within 10 days. If you
haven’t done so within 10 days, you can still submit a claim in writing. - Must see a company approved doctor.
- You are entitled to a one-time change of physician. This change must happen within 90 days of
the date of injury. The new doctor must be another physician on the designated provider list. You
must request the change in writing to your case manager. This is a one-time change and cannot be
contested by the employer. If you wish to change again, you may ask in writing, but they can
deny your request. - There is no minimum payment per week, but the maximum is $1,293.25 a week.
- There is no temporary total disability payments the first 3 days you are pulled from work. After
you are off work 14 days, they will go back and pay you for the first 3 days you missed. - You can be reimbursed for your mileage to/from doctors’ appointments at the rate of .59 cents per
mile. Ask your case manager for more information. - You can reopen your previous claim 2 years from the last date of payment or 6 years from the

Fort Lauderdale
- You must report your injury to your employer within 30 days of the date of injury, or within 30
days of a doctor telling you are suffering from an on the job injury. - You must see a doctor that is within the company’s network.
- The workers comp pay in Florida is a maximum weekly pay: $1,197.
- You will not be paid for the first 7 days of your injury until you are out for more than 21 days.
Then you will receive the first 7 days pay. - You can receive wage replacement benefits for a maximum of 104 weeks in Florida.

Las Vegas
- You must report the injury in writing within 7 days of the injury.
- You need to see the company doctor within 90 days of the injury date.
- Must see a company approved doctor. You can choose a doctor from the Nevada Panel of doctors
list. - The first 5 days the doctor keeps you out of work are unpaid. You will start receiving

20 or more miles one way to seek medical treatment. The mileage reimbursement rate is 62.5

Los Angeles
- You can choose to see your own doctor in CA if you have pre-designated a physician of your
choosing before you are injured. - Your pre-designation form must be on file with the base or Attendance and leave before your
injury occurs. - You must report your injury to your employer in writing within 30 days of the date of injury. If
you do not, you could lose your right to receive workers compensation benefits. - The temporary total disability state maximum weekly pay: $1,651.
- You must be taken out of work for at least 3 consecutive days before receiving temporary total
disability payments. Once you are out for 14 consecutive days you will then receive pay for the
first 3 days. - You are entitled to mileage reimbursement. Your case manager can send you the mileage

Nashville
- You must report the injury in writing within 15 calendar days or you could be denied workers compensation.
- Per TN law, you can only be treated by a doctor within the company’s network of physicians.
- You can be reimbursed for mileage if you travel more than 15 miles one way for injury related appointments. You can be reimbursed at 65.5 cents per mile.
- You are not paid by workers compensation unless you are pulled from work for more than 7 days. If you are pulled from work more than 14 days, you will be paid for the first 7 days.
- The minimum pay per week is $179.10. The maximum pay per week is $1,313.40.

Chicago
- You must report the injury in writing to the company within 45 days of the injury date.
- You can choose to see your own doctor in Illinois. The doctor of your choice must take workers
compensation. Before seeing your own doctor for your injury give your case manager your
doctor’s name and information. - Temporary total disability payments are governed by state law. Maximum weekly pay: $1,848.20.
- Temporary total disability payments don’t start until you have been kept off work by the doctor
for 3 consecutive days. Once you have been out for 14 consecutive days, your case manager will
go back and pay you for the first 3 days. - You can request mileage reimbursement for travel to/from medical treatments. The rate of pay is

Phoenix
- Claims must be filed within 1 year of injury date.
- You must see the company doctor first. After that initial visit you can choose a treating physician
that accepts workers’ compensation claims. However, once you see the same doctor twice, that is
your treating physician. Your case manager must approve any other physician changes. - The first 7 days you are off work due to injury are unpaid. If you are off work 14 days are more
then you are paid for the first 7 days off work. - Maximum pay: $5,393.37 per month.
- Mileage reimbursement is 62.5 cents per mile. Please contact your case manager for more
information. - Arizona offers free help with your claim. The office of the Ombudsman is available to answer
